The Bridging the Transition Framework

What is the Bridging the Transition Framework?

The Bridging the Transition Framework is a framework to guide homelessness prevention and was initially developed to guide the practice and research of a growing number of occupational therapists who were supporting individuals across the trajectory of unhoused to housing. We developed the framework initially in 2018 through funding provided by the Ontario Society of Occupational Therapists. In 2022, we were able to secure additional funding from the Social Sciences and Humanities Research Council of Canada to further refine and disseminate this framework. This funding enabled our team to conduct research to provide a foundation for developing the framework, and refine the framework in 2023 and further in 2025. Since the first release of the framework, we have recognized its relevance for a range of professions outside of occupational therapy, and we have since changed the language in the framework, written our book for an interdisciplinary audience, and revised our workshop so that it would be relevant for a range of professions.
